If possible, for our Fayetteville office please mail the forms to Village Surgical Associates, PA, 1841 Quiet Cove, Fayetteville, NC 28304 or you may fax the forms to (910-323-3862). For our Erwin/Dunn office, please mail the forms to Village Surgical Associates, PA, 133 Professional Park, Erwin, NC, 28339 or you may fax the forms to (910-892-1992). You may also drop the forms off at either location before your appointment. This will assist us with your consultation on the day of your appointment. Otherwise, plan on arriving 30 minutes prior to your scheduled appointment or consultation.
